Job summary

SERENITY HOME HEALTH CARE in Rockford, IL seeks a Care Coordinator to manage clients through their lifecycle, handle intake calls, and provide information on services and fees. This role ensures quality care and accurate client and employee records.

You will be the main contact for clients and partners, coordinate care plans with caregivers, monitor daily activities, and maintain timely documentation in client files.

Qualifications

• Demonstrated customer service, communication skills, problem solving, and attention to detail.
• Technical proficiency with computers and generating reports.
• Fluent spoken/written English; Spanish is a plus.

Responsibilities

• Serve as first contact for clients, industry partners, and care providers, delivering excellent customer service.
• Handle incoming phone inquiries, provide service/fee information, and gather client care data.
• Monitor daily dashboard and coordinate with caregivers and clients as needed.
• Share on-call duties on a rotating monthly basis.
• Coordinators input care plans, ensure staffing, and prepare caregivers for start of care.
• Complete monthly client calls and document in the client file.
