Job summary

SERENITY HOME HEALTH CARE is seeking a Client Coordinator in South Holland, IL to ensure high-quality, timely, and coordinated care for assigned clients. You will oversee onboarding, care plan management, scheduling, communication, and thorough documentation to support seamless service delivery.

You will act as the main point of contact for clients, caregivers, and internal teams, ensuring timely responses and adherence to payer requirements and organizational standards.

Qualifications

• High school diploma or equivalent required.
• 1–2 years client-facing experience in home care or healthcare preferred.
• Demonstrated customer service, communication, empathy, and problem-solving skills.

Responsibilities

• Oversee client onboarding, care plan management, scheduling, and documentation.
• Serve as the primary contact for clients, caregivers, and internal departments.
• Verify and maintain accurate client profiles and service records.
• Process updates to care plans and scheduling within required timelines.
• Ensure timely communication with supervisors about changes in client status or care needs.
• Maintain a clear schedule for all active clients on the coordinator’s dashboard.
• Document all client-related communications to ensure compliance.
• Conduct weekly outreach to clients with services on hold to assess needs.
• Review missed visits and arrange makeup hours to maintain continuity.
• Close client profiles promptly after service termination with documented rationale.
