Job summary

Qualifications

• High school diploma or equivalent.
• At least 2 years in a supervisory role.
• Fluent in English and Spanish, both written and spoken.

Responsibilities

• Serve as first point of contact for clients, partners, and care providers, delivering excellent customer service.
• Answer phone inquiries, explain services/fees, and collect information for care plans.
• Monitor daily dashboard; follow up with caregivers and clients.
• Share on-call duties every other month.
• Coordinators input care plans and review staffing with caregivers.
• Complete client monthly calls and document in files.
• Update care plans and upload monthly reports; communicate changes.
• Match caregivers with clients for best fit.
• Ensure care plan compliance and proper documentation.
• Attend staff trainings and meetings.
• Perform clerical tasks (filing, uploading, copying, faxing).
• Other duties as assigned.
