Qualifications

• Demonstrated customer service and communication skills with problem solving and attention to detail.
• Technical proficiency in utilizing computers and generating reports.
• Able to speak, read and write English & Spanish fluently.
• Ability to speak other languages is an advantage.

Responsibilities

• Be the first contact for clients, partners and care providers with excellent service.
• Take phone inquiries, provide service and fee information, gather care details.
• Daily dashboard monitoring and follow-up with caregivers and clients.
• Occasional on-call duties every other month.
• Coordinators input care plans and review with assigned caregiver before start.
• Complete monthly client calls and document in client files.
• Update care plans, upload monthly reports, and communicate changes.
• Match caregivers to clients for best fit and document changes.
• Ensure compliance with care plans and participate in trainings/meetings.
• Perform clerical tasks and other duties as assigned.
