Job summary

ROWI Teen & Parent Wellness Centers is seeking a Care Coordinator to oversee coordination of care for clients, including orientation, transportation, appointments, and scheduling. You will ensure attendance at groups, facilitate activities, and document consistently.

This role requires a high school diploma (Bachelor’s preferred), plus 2 years driving and 1 year working with youth in a therapeutic setting. A professional, compassionate approach is essential for success at ROWI.

Qualifications

• High school diploma required; Bachelor’s degree preferred.
• Minimum 2 years driving experience.
• At least 1 year of experience working with kids in a therapeutic setting.

Responsibilities

• Coordinate individualized services including orientation, transportation, doctor visits, and scheduling.
• Ensure clients attend designated groups and follow program schedules.
• Facilitate groups and activities as assigned by the Facility Manager.
• Conduct client orientation and necessary assessments at admission.
• Maintain compliance with Joint Commission Standards.
• Document shift reports and EMR entries within 24 hours.
