ROWI Teen & Parent Wellness Centers in Visalia is seeking a Care Coordinator to support coordination of care for all ROWI clients under the supervision of the Program Director. You will coordinate individualized services including orientation, transportation, doctor visits, and scheduling.

The role requires a high school diploma (Bachelor’s preferred), at least two years of driving experience, and experience with kids in a therapeutic setting.

Formación

• High school diploma required; Bachelor’s degree preferred.
• Minimum 2 years driving experience.
• Minimum 1 year working with kids in a therapeutic setting.
• Experience with junior high or high school students is preferred.

Responsabilidades

• Coordinate individualized care including orientation, transportation, doctor visits, and scheduling.
• Ensure programs and services are implemented to high standards and clients receive quality care.
• Supervise clients and ensure attendance at designated groups.
• Conduct client orientation on admission day and perform/coordinate assessments as directed.
• Document shift reports and share pertinent information with the team.
• Coordinate transportation schedules and drive clients as needed.
• Document all transportation and group activity in EMR; submit documentation within 24 hours.
• Maintain confidentiality and professional boundaries; comply with dress code.
