Job summary

ROWI Teen & Parent Wellness Centers in San Diego, CA seeks a Care Coordinator to manage care coordination for clients, including orientation, transportation, doctor visits, and scheduling.

Under supervision of the Program Director, you will document in EMR, ensure standards, coordinate groups, and maintain professional, confidential communication with families and staff. This role requires a high school diploma (Bachelor’s preferred) and at least two years driving experience.

Qualifications

• High school diploma required; bachelor’s degree preferred.
• Minimum 2 years of driving experience.
• Minimum 1 year of work with kids in a therapeutic setting is required.
• Experience with junior high or high school students preferred.

Responsibilities

• Coordinate care for ROWI clients, including orientation and scheduling.
• Manage transportation schedules and drive clients as needed.
• Document all activities and EMR entries within 24 hours.
• Ensure compliance with care standards and confidentiality.
• Support group activities and client supervision.
• Maintain professional communication with families and staff.
