Job summary

ROWI Teen & Parent Wellness Centers in California is seeking a Care Coordinator under the supervision of the Program Director to coordinate care for ROWI clients, including orientation, transportation, doctor visits, and scheduling.

This role requires strong communication, time management, and ability to work within an interdisciplinary team. You will facilitate groups, document in EMR, ensure program standards, and support families throughout treatment.

Qualifications

• Minimum of a high school diploma; Bachelor’s degree preferred.
• Minimum of two years driving experience.
• Minimum of 1 year working with kids in a therapeutic setting.
• Experience with junior high or high school students preferred.

Responsibilities

• Coordinate care including orientation, transportation, doctor visits, and scheduling.
• Ensure programs and services are implemented at the highest standards and clients receive quality care.
• Supervise clients and ensure attendance at designated groups throughout programming.
• Facilitate assigned groups and activities as designated by the Facility Manager.
• Conduct client orientation on admission day and perform or assign assessments as directed.
• Document shift reports and share pertinent information with the team.
• Coordinate transportation schedule for all clients and drive clients as needed.
• Document transportation and group activity in EMR; submit documentation within 24 hours.
• Assist with facility cleanliness and coordinate daily client meals.
• Maintain confidentiality and professional boundaries with clients and staff.
• Attend staff and in-service meetings; comply with dress code.
