Job summary

Socket.dev is seeking a Care Coordinator to support ROWI clients with care coordination, orientation, transportation, and scheduling under the supervision of the Program Director.

The role emphasizes EMR documentation, group facilitation, driving responsibilities, and maintaining professional conduct within an interdisciplinary team environment. A high school diploma is required; a Bachelor’s degree is preferred.

Qualifications

• Must have at least a high school diploma; Bachelor's degree preferred.
• Minimum of two years driving experience required.
• Minimum of 1 year working with kids in a therapeutic setting.
• Experience with junior high or high school students is preferred.

Responsibilities

• Coordinate individualized service including orientation, transportation, doctor visits, and scheduling.
• Ensure programs and services are implemented at the highest standards and clients receive the highest level of care.
• Document shift reports and share pertinent information with the team.
• Coordinate the transportation schedule for all clients and drive clients as needed.
• Document all transportation and group activity in EMR.
• Assist with facility cleanliness as needed.
• Maintain the confidential nature of clients and related activities.
• Attend designated staff meetings and in-service trainings.
• All documentation in EMR must be submitted within 24 hours.
