Job summary

Socket.dev is seeking a Care Coordinator to supervise and coordinate care for ROWI clients, including orientation, transportation, and doctor visits. The role involves ensuring client participation in groups and maintaining EMR documentation with 24-hour turnaround.

You will coordinate schedules, drive clients as needed, and uphold confidentiality while collaborating with an interdisciplinary team in a compassionate, professional manner.

Qualifications

• Must have at least a high school diploma; Bachelor's degree preferred.
• Minimum of 2 years driving experience required.
• Minimum of 1 year working with kids in a therapeutic setting.
• Experience with junior high or high school students is preferred.

Responsibilities

• Coordinate individualized service including orientation, transportation, doctor visits, and scheduling.
• Ensure programs and services are implemented to high standards; clients receive quality care.
• Supervise clients and ensure attendance at designated groups.
• Conduct client orientation on admission day or assign to staff as needed.
• Maintain and document all shift reports and pertinent team information in EMR.
