Job summary

WellLife Network in Glendale, NY is seeking a Direct Care Aide to accompany persons served to medical appointments and advocate for needed services. You will provide direct care and act as liaison between residence and medical facilities, ensuring appointments are carried out.

This role requires a High School diploma, an NYS driver's license, and experience with developmentally disabled individuals or medical settings.

Qualifications

• High School diploma or equivalent is required.
• Associate’s degree in Human Services preferred.
• Experience working with developmentally disabled individuals and/or medical field experience.
• A valid NYS Driver's license is required.

Responsibilities

• Accompany persons served to medical appointments and advocate for appropriate medical services.
• Ensure medical appointments and necessary follow-ups are carried out; liaise between residence and providers.
• Safely operate agency vans and/or vehicles as per policy; document medical information in EHR.
• Train persons served in activities of daily living and support self-help and social skills; assist with ADL tasks.
• Maintain confidentiality and participate in regular team meetings to review health plans.
