Job summary

WellLife Network in New York seeks a compassionate caregiver to accompany individuals served to medical appointments and advocate for necessary services. The role includes direct care, liaison between residence and medical facilities, and documenting visits using the EHR system.

Computer use is required and confidentiality must be maintained. You will work with a team to ensure follow-up medical care, assist with activities of daily living, and support medication administration while

Qualifications

• High School diploma or equivalent required; Associate’s degree in Human Services preferred.
• Experience working with the developmentally disabled and/or experience in the medical field.
• A valid NYS Driver's license is required.

Responsibilities

• Accompany persons served to medical appointments and advocate for appropriate medical services.
• Provide direct care services and liaise between residence and medical facilities, hospitals, laboratories and pharmacies.
• Ensure medical, dental or other therapeutic appointments are carried out as required; use computer for documentation.
