Job summary

WellLife Network is seeking a direct care worker to accompany individuals to medical appointments and advocate for needed services in NY. The role includes providing direct care, assisting with ADLs, and documenting medical information in the EHR system. A valid NYS driver's license and experience with developmentally disabled individuals are preferred.

You'll work in a supportive, team-based environment with ongoing training, benefits, and opportunities to grow within the organization.

Qualifications

• High School diploma or equivalent required.
• Associate's degree in Human Services preferred.
• Experience working with developmentally disabled individuals recommended.
• Valid NYS Driver's license required.

Responsibilities

• Accompany persons served to medical appointments and advocate for appropriate services.
• Provide direct care and assist with activities of daily living.
• Operate agency vehicles safely per policy.
• Document medical information and interventions in the EHR/EMAR systems.
• Communicate medical concerns to Residence Manager/Nurse and participate in reviews.
• Support training in daily living activities and self-help skills.
