Zusammenfassung

WellLife Network is seeking a dedicated caregiver to accompany individuals to medical appointments, advocate for needed services, and provide direct care within a nonprofit health and human services setting in New York. The role supports residents with daily activities, documentation, and coordination with medical providers.

Ideal candidates have a High School diploma or equivalent, a valid NYS driver’s license, and relevant certifications.

Qualifikationen

• High School diploma or equivalent required.
• Associate’s degree in Human Services preferred.
• A valid NYS Driver's license is required.

Aufgaben

• Accompany persons served to medical appointments and advocate for appropriate medical services.
• Provide direct care services and act as liaison between residence and medical facilities.
• Ensure medical, dental or other therapeutic appointments are carried out as required.
• Operate agency vans and/or vehicles as per policy.
• Communicate immediate medical concerns to Residence Manager/Nurse.
• Participate in weekly consult review meetings.
• Document medical interventions and tests in binders and EHR.
• Assist in training in ADL and development of self-help skills.
• Help maximize independence through budgeting, nutrition, transportation, and home activities.
• Manage medication documentation in EMAR system as applicable.
• Communicate information to staff; complete logs and incident reports.
• Physically intervene in dangerous situations and administer first aid when needed.
• Complete AMAP, SCIP, CPR and First Aid techniques.
• Maintain confidentiality at all times.
