Job summary

WellLife Network in New York seeks a dedicated caregiver to accompany residents to medical appointments and advocate for needed services. You will provide direct care, liaise with hospitals and clinics, and ensure follow-up is completed.

The role requires assisting with daily living activities, documenting in the EHR, and maintaining confidentiality. A NYS driver’s license is required along with relevant experience.

Qualifications

• High School diploma or equivalent required.
• Associate’s degree in Human Services preferred.
• Experience with developmentally disabled and/or medical field.
• Valid NYS Driver’s license required.

Responsibilities

• Accompany persons served to medical appointments and advocate for services.
• Provide direct care and liaise with medical facilities, hospitals, laboratories and pharmacies.
• Ensure medical, dental or other therapeutic appointments are carried out as required.
• Safely operate agency vans and/or vehicles per the agency policy.
• Document medical information and interventions in the EHR system.
• Assist in training persons served in activities of daily living and self-help skills.
• Maintain confidentiality at all times.
• Participate in weekly consult review meetings with Residence Manager and Residence Nurse.

Education
