Job summary

Concierge Solutions (Mobile Medical) in Port St Lucie, FL, is seeking caring healthcare professionals to deliver chronic care management and preventive services in patients’ homes. You will document care, coordinate with providers, and build detailed care plans to guide interactions and transitions.

Ideal candidates have 1+ year in homecare or primary care, CNA/HHA credentials, and strong EMR skills. We offer PTO, bonuses, 401(k), and health benefits in a supportive team environment.

Qualifications

• 1 year of homecare, Skilled Nursing Facility, or Primary Care experience.
• Knowledge of medical terminology and interventions.
• Computer/EMR system savvy.

Responsibilities

• Document preventive care; coordinate and complete care.
• Provide chronic care management for geriatric patients via calls and visits.
• Create and maintain detailed care plans to guide interactions.
• Make calls to CCM-enrolled patients.
• Coordinate with tertiary care providers and manage referrals.
• Use HIPAA-compliant systems to facilitate care.

Location: This position is based in Port St Lucie, FL

• Ensure all preventive care is documented, coordinated, and completed.
• Provide chronic care management for geriatric patients via telephone calls and visits.
• Create and maintain detailed care plans to guide patient interactions and care.
• Make phone calls to patients enrolled in Chronic Care Management (CCM) programs.
• Support continuity of care by coordinating with tertiary care providers, managing care transitions, and handling referrals.
• Use HIPAA-compliant computer systems to facilitate patient care.

Qualifications:

• Certified Nursing Assistant preferred, but will also consider Home Health Aide (HHA), Medical Assistant certification or higher
• Minimum 1-year experience in homecare, Skilled Nursing Facility or Primary Care setting
• Minimum 1-year experience in Chronic Care Management preferred
• Knowledge of medical terminology and disease specific interventions
• Computer/EMR system savvy
