Job summary

Concierge Home Care in Jacksonville, FL is seeking a Scheduling Coordinator to support our clinical team and coordinate patient visits with WellSky (Kinnser) EMR. This in-office role focuses on organizing schedules, communicating with clinicians, and ensuring timely patient care.

You will manage daily scheduling, coordinate clinician availability, and handle urgent visit requests while maintaining accurate records and supporting RAP/EOE processes.

Qualifications

Responsibilities

• Scheduling patient visits in accordance with physician orders and care plans.
• Coordinating clinician schedules to optimize productivity and coverage.
• Managing daily scheduling changes, call-outs, and urgent visit needs; answering and triaging incoming calls.
• Tracking medical supplies and supporting RAP/EOE processes.
• Maintaining accurate scheduling and documentation within the EMR.
• Supporting the clinical team with administrative tasks as needed.

Your Role as Clinical Team Support

In this role, you will support daily clinical operations by coordinating schedules and ensuring seamless communication between patients and clinicians.

Responsibilities include:

Schedule: Monday–Friday, 8:30 AM – 5:00 PM (in-office role)

Compensation: negotiable,based on experience
