Job summary

Mobile Medical in Jacksonville, FL seeks a Patient Care Navigator to coordinate post-visit services, manage orders, and ensure patients receive timely home health, lab work, DME, radiology, and medications. You’ll support care continuity and documentation while partnering with clinicians to improve patient outcomes.

Ideal candidates have at least one year in a medical office setting, strong communication skills, and proficiency with Microsoft Office.

Qualifications

  • Minimum one year of medical office experience.
  • Experience with front office operations (check-in/check-out).
  • Medical Assistant certification preferred.
  • Strong organizational and multitasking abilities.
  • Ability to work in a high-call, fast-paced environment.
  • Excellent customer service and communication skills.
  • Proficiency with Microsoft Office.

Responsibilities

  • Process provider orders and follow-up tasks after visits.
  • Coordinate services such as home health, laboratory testing, DME, and radiology.
  • Complete prior authorizations for medications and services.
  • Process medication refills and communicate updates with providers.
  • Collaborate with the healthcare team to ensure continuity of care.
  • Maintain compliance with medical record documentation standards.
  • Communicate with patients, pharmacies, and care partners to coordinate services.
  • Support updates related to home health and other care services.
  • Health insurance begins after your first 30 days of employment.

Your Role as a Patient Care Navigator
