Job summary
La Frontera EMPACT - Suicide Prevention Center in Arizona is seeking a Children's Case Manager for the City of Maricopa. The role includes direct crisis intervention, transport and education services to families and children, and coordinating care with partner agencies.
Full-time with flexible scheduling to meet client needs. Qualifications include a Bachelor’s in Behavioral Health or related field, experience with the Child and Family Team process, fingerprint clearance, valid Arizona driver
Qualifications
- Bachelor's degree in Behavioral Health field or related experience preferred.
- Bi-lingual Spanish is preferred.
- Must have Fingerprint clearance or be eligible to obtain one.
- Must have a valid AZ driver's license with clean record.
Responsibilities
- Provide direct crisis intervention, support, transportation and education to families and children.
- Complete family needs assessments and make referrals to partners and community services.
- Coordinate communications with other departments and collaborators to ensure continuity of care.
