Child & Family Development is seeking a dedicated professional to work with East Bay families, helping them access services, resources, advocacy, education, or skills through Wraparound Services. This hybrid role allows 20% remote work while engaging families in their homes and communities.

The position emphasizes applying the Wraparound model, collaborating with a team, and ensuring data collection and documentation align with program requirements. A background in social work is preferred.

Competenze

• Bachelor's degree in social work or related field required.
• Experience in child welfare, parent education, home-based services, and/or crisis intervention required.
• Travel along the East Bay is required.

Mansioni

• Learn and utilize the national Wraparound Model to assist families in meeting their goals.
• Use the 10 principals of Wraparound to guide your work and engagement with families.
• Work closely with a team of colleagues who meet with families in their homes and communities.
• Help families identify natural support and participate as members of the Wraparound Team while adhering to HIPAA and confidentiality guidelines.
• Learn families' strengths and needs through the lens of their culture and values.
• Responsible for ongoing data collection, evaluation, and assessment of the Family Care Plan, plus timely documentation.
• Work with parents on improving child management skills through modeling, education, support, and coping skills.
• Coordinate services with schools, healthcare providers, court systems, and other service providers.
• Participate in community outreach events to educate the community about FCCP as a resource.
