Job summary

Qualifications

  • Experience providing Wraparound facilitation, care coordination, family-centered planning, FGDM, case management, or services to children, youth, and families with complex needs is preferred.
  • Knowledge of children's mental health, community resources, educational systems, human services systems, and collaborative service delivery.
  • Strong interpersonal, communication, facilitation, and organizational skills, with the ability to build rapport with families, natural supports, and service providers from diverse backgrounds.
  • Demonstrated ability to establish effective partnerships, promote family voice and choice, support collaborative problem-solving, and facilitate planning processes that improve outcomes for children, youth, and families.

Responsibilities

  • Facilitate MiiWrap, Wraparound, FGDM, and other family-centered planning processes to support children, youth, and families with complex needs.
  • Conduct assessments and partner with families to develop individualized plans of care.
  • Convene and facilitate Child and Family Team meetings and other planning meetings to monitor progress and coordinate services.
  • Coordinate multidisciplinary teams, ensuring effective communication, collaboration, accountability, and follow-through among team members.
  • Collaborate with the Family Liaison to support implementation of plans and strengthen continuity of care.
  • Support development of transition and closing plans that identify ongoing services and natural supports.
  • Monitor fidelity to Wraparound and family-centered planning models and support continuous quality improvement.
  • Maintain accurate documentation, assessments, plans of care, progress notes, and required program records.
  • Develop and maintain collaborative relationships with schools, county agencies, healthcare organizations, mental health providers, community organizations, and family-run organizations.
  • Participate in required meetings, supervision, training, quality improvement activities, and professional development opportunities.
  • Perform other duties as assigned.

JOB SUMMARY: The Wraparound Coordinator facilitates family-driven, team-based planning processes designed to improve outcomes for children, youth, and families with complex needs. Using the principles of MiiWrap, Wraparound, Family Group Decision Making (FGDM), and other family-centered planning approaches as appropriate, the Wraparound Coordinator partners with families, natural supports, and service providers to identify strengths, develop individualized plans, coordinate services, and support achievement of family goals. The position is responsible for maintaining fidelity to approved family-centered planning models through ongoing training, coaching, documentation, and continuous quality improvement activities.

The Wraparound Coordinator serves as the facilitator and coordinator of Child and Family Teams, ensuring that planning processes remain family-driven, strengths-based, culturally responsive, and focused on measurable outcomes. The position works closely with the Family Liaison and community partners to strengthen care coordination, improve continuity of care, support successful transitions, and promote long-term family stability and independence.

SPECIFIC RESPONSIBILITIES:
  • Facilitate MiiWrap, Wraparound, Family Group Decision Making (FGDM), and other family-centered planning processes as appropriate to support children, youth, and families with complex needs.
  • Conduct assessments and partner with families, natural supports, and service providers to identify strengths, needs, goals, and priorities and develop individualized plans of care.
  • Convene and facilitate Child and Family Team meetings and other planning meetings to monitor progress, address barriers, coordinate services, and support achievement of identified goals.
  • Coordinate multidisciplinary teams, ensuring effective communication, collaboration, accountability, and follow-through among team members.
  • Collaborate closely with the Family Liaison to support implementation of plans, reinforce family engagement, monitor progress, and strengthen continuity of care throughout service delivery and aftercare.
  • Support development of transition and closing plans that identify ongoing services, natural supports, crisis response strategies, and follow-up activities to promote long-term success.
  • Monitor fidelity to Wraparound and family-centered planning models and support continuous quality improvement activities.
  • Maintain accurate documentation, assessments, plans of care, progress notes, and required program records in accordance with agency, state, and grant requirements.
  • Develop and maintain collaborative relationships with schools, county agencies, healthcare organizations, mental health providers, community organizations, and family-run organizations to strengthen coordinated systems of care.
  • Participate in required meetings, supervision, training, quality improvement activities, and professional development opportunities.
  • Perform other duties as assigned.
QUALIFICATIONS, KNOWLEDGE, SKILLS, AND EXPERIENCE:
  • Bachelor's degree in social work, psychology, human services, education, public health, or a related field required. Relevant experience may be considered in lieu of a degree. Experience providing Wraparound facilitation, care coordination, family-centered planning, Family Group Decision Making (FGDM), case management, or services to children, youth, and families with complex needs is preferred.
  • Knowledge of children's mental health, community resources, educational systems, human services systems, and collaborative service delivery. Understanding of family-driven, strengths-based, culturally responsive, and team-based planning approaches.
  • Strong interpersonal, communication, facilitation, and organizational skills, with the ability to build rapport with families, natural supports, and service providers from diverse backgrounds. Ability to coordinate multidisciplinary teams, manage competing priorities, maintain accurate documentation, and utilize technology for communication, recordkeeping, and reporting.
  • Demonstrated ability to establish effective partnerships, promote family voice and choice, support collaborative problem-solving, and facilitate planning processes that improve outcomes for children, youth, and families.
SALARY:

This is a full-time position with comprehensive benefits. Salary will range from $51,277 depending on experience, education and training.

TRAVEL:

Travel within PACT's five-county service region. Some travel within the state.
