The National Child Traumatic Stress Network (NCTSN) seeks a Trauma Care Case Manager to participate in SAMHSA-funded NCTSI grant – Centerstone Trauma and Grief Treatment for Children, Adolescents, and Families. Provide trauma care case management for children aged 0–17 who have endured trauma, within Manatee, Sarasota and DeSoto counties.

Qualifications include a Bachelor’s degree in social work, psychology or related field with at least two years’ trauma experience; bilingual preferred.

Responsabilidades

• Coordinate referrals and follow-up to meet individual or youth/family needs.
• Develop, maintain, and utilize community referral sources.
• Screen for eligibility and co-occurring disorders.
• Provide referrals for ACA enrollment.
• Ensure that identified individuals receive needed assistance and support services.
• Track treatment plan progress and trauma/risk status.
• Provide trauma-related education and preventative care.
• Provide immediate assistance to clients/families in crisis.
• Attend meetings and coordinate with community organizations.
• Complete intake, needs assessment and treatment planning.
• Attend multi-disciplinary team or community at-risk meetings.
