Qualifications

• Associate degree is preferred.

Responsibilities

• Collects data and contributes to assessment, planning and evaluation of individualized care.
• Provides direct care with RN support and as a team.
• Facilitates psychoeducational and therapeutic activity groups.
• Documents in the medical record per policy.
• Encourages activities of daily living and maintains a safe environment.
