Family Health Centers of San Diego is seeking a Patient Health Navigator to provide direct and indirect patient services, support referrals, and maintain care linkage for assigned programs within a diverse, multi-disciplinary team.

You will engage with patients in person and by phone, document with precision, assist with scheduling and care coordination, and help remove barriers to care while following safety and privacy guidelines.

Qualifikationen

• High School diploma or GED required.
• CA HIV Test Counselor or phlebotomy-related certifications may be required or preferred depending on assignment.

• Complete required documentation accurately and timely per department standards.
