Job summary

Family Health Centers of San Diego is seeking a Patient Health Navigator to work with the Tiger team on home based medical services. The role involves coordinating visits, managing schedules, and helping patients overcome barriers to care.

You will gather patient information, liaise with the team, and ensure timely documentation and referrals. The position emphasizes teamwork, customer service, and accurate data entry within EMR systems, with travel across San Diego County and bilingual

Qualifications

• High School diploma or GED required.
• 2 years health education or medical field experience preferred.
• Ability to travel within San Diego County; valid CA driver's license may be required.

Responsibilities

• Help coordinate and schedule daily visits for the Tiger team responsible for home based medical services.
• Create patient home visit schedules to maximize team productivity.
• Manage multiple patient schedules including cancellations and events.
• Assist patients in addressing barriers to care and connecting to referrals.
• Gather information from the Tiger team and patients to schedule visits via phone, in person or electronically.
• Provide responsive assistance to patient and family needs and coordinate plan with the team.
• Operate scheduling templates and adjust as needed; support reporting on outreach and visits.
• Coordinate lab courier schedules and ensure EMR demographics are up to date.
• Deliver excellent customer service and complete all required documentation accurately.
