Job summary

Family Health Centers of San Diego seeks a Health Navigator to support linkage to and maintenance in care across assigned programs. Under limited supervision, you will handle referrals, collaborate with a multidisciplinary team, and contribute to quality improvement projects.

You will engage with patients via phone and in person, complete documentation, and assist with scheduling, data entry, and other duties while protecting confidentiality.

Qualifications

• High School diploma (or GED) required.
• 2 years of experience delivering health education, preferably in a community health setting.
• Specific Certifications may be required depending on assignment.

Responsibilities

• Acts as a liaison between patients, staff and other service providers to improve patient care and flow.
• Completes all required documentation accurately and timely in accordance with standards.
• Works in collaboration with healthcare team on retention in care and appointment completion, and identifies barriers to care.
• Engages in patient interactions by telephone and in person to identify needs and provide referrals and follow-up.
• Performs general clinical tasks such as filing, data entry, scheduling, and related duties.
