Autumn View Health Care Facility, part of The McGuire Group, seeks a Certified Occupational Therapy Assistant (COTA) to join the rehabilitation team in Buffalo, NY. You will work under an OT to deliver patient-centered interventions and help residents regain function and independence.

Located in Hamburg and Buffalo, the facility prides itself on quality care and collaborative growth. Strong communication and a NY COTA license are required, with newly licensed COTAs welcome to apply.

Qualifications

Responsibilities

• Implement occupational therapy treatment programs under supervision.
• Assist with resident assessments and contribute to treatment planning within the COTA scope.
• Provide therapeutic interventions focused on improving ADLs, mobility, and independence.
• Monitor resident progress and communicate findings to the supervising Occupational Therapist.
• Complete documentation, care plans, billing, and clinical records accurately and timely.
• Educate residents, families, and staff on therapy techniques and home exercise programs.
• Train and support rehabilitation aides as assigned.
• Participate in interdisciplinary meetings, discharge planning, and quality improvement initiatives.
• Ensure residents receive prescribed therapy services and treatment minutes.

RESPONSIBILITIES:

• Educate residents, families, and staff on therapy techniques, adaptive equipment, and home exercise programs
• Train and support rehabilitation aides as assigned
• Participate in interdisciplinary meetings, discharge planning, and quality improvement initiatives
• Ensure residents receive prescribed therapy services and treatment minutes
• Perform additional duties as assigned

REQUIREMENTS:

• Associate of Applied Science Degree from an Occupational Therapy Assistant program approved by the American Occupational Therapy Association
• Valid New York State Certified Occupational Therapy Assistant (COTA) License required
• Newly licensed COTAs welcome to apply
• Skilled nursing facility experience preferred
• Strong communication, documentation, and interpersonal skills
• Ability to work collaboratively with residents, families, therapists, and interdisciplinary team members
