Job summary

Autumn View Health Care Facility in Hamburg, NY, part of The McGuire Group, is seeking a Certified Occupational Therapy Assistant (COTA) to join its rehabilitation team. You will work under the supervision of an Occupational Therapist to provide therapeutic interventions to help residents improve function, maintain independence, and achieve their highest level of performance.

Applicants should hold a NY State COTA license, have an AAS in OT, and be committed to resident-centered care.

Qualifications

• Associate of Applied Science Degree from an OT AOTA-approved program.
• New York State COTA license (required).
• Newly licensed COTAs welcome to apply.
• Skilled nursing facility experience preferred.
• Strong communication, documentation, and interpersonal skills.
• Ability to work collaboratively with residents, families, therapists, and interdisciplinary team.

Responsibilities

• Implement occupational therapy treatment programs under supervision.
• Assist with resident assessments and contribute to treatment planning.
• Provide interventions to improve functional abilities, ADLs, mobility, strength, and independence.
• Monitor resident progress and communicate findings to supervising OT.
• Complete documentation, care plans, billing, and records accurately and timely.
• Educate residents, families, and staff on therapy techniques and home exercise programs.
• Train rehabilitation aides as assigned.
• Participate in interdisciplinary meetings, discharge planning, and quality improvement initiatives.
• Ensure residents receive prescribed therapy services and treatment minutes.

RESPONSIBILITIES:

• Provide therapeutic interventions focused on improving functional abilities, ADLs, mobility, strength, and independence
• Monitor resident progress and communicate findings to the supervising Occupational Therapist
• Complete documentation, care plans, billing, and clinical records accurately and timely
• Educate residents, families, and staff on therapy techniques, adaptive equipment, and home exercise programs
• Train and support rehabilitation aides as assigned
• Participate in interdisciplinary meetings, discharge planning, and quality improvement initiatives
• Ensure residents receive prescribed therapy services and treatment minutes
• Perform additional duties as assigned

REQUIREMENTS:

• Associate of Applied Science Degree from an Occupational Therapy Assistant program approved by the American Occupational Therapy Association
• Valid New York State Certified Occupational Therapy Assistant (COTA) License required
• Newly licensed COTAs welcome to apply
• Skilled nursing facility experience preferred
• Strong communication, documentation, and interpersonal skills
• Ability to work collaboratively with residents, families, therapists, and interdisciplinary team members
