Job summary

Chautauqua Nursing and Rehabilitation Center in Dunkirk, NY seeks a Certified Occupational Therapy Assistant (COTA) to join the rehabilitation team. You will work under an Occupational Therapist to deliver interventions that help residents improve function, independence, and performance.

You will implement treatment programs, participate in assessments, and educate residents and families on therapy techniques and home programs. Prior SNF experience is preferred and NY COTA licensure is required.

Qualifications

• AAS degree in Occupational Therapy Assistant (or equivalent).
• New York State COTA license required.
• Newly licensed COTAs welcome to apply.

Responsibilities

• Implement OT treatment programs under the supervision of an Occupational Therapist.
• Assist with resident assessments and contribute to treatment planning within the COTA scope of practice.
• Provide interventions to improve functional abilities, ADLs, mobility, and independence.
• Monitor resident progress and communicate findings to the supervising OT.
• Complete documentation, care plans, and clinical records accurately and timely.
• Educate residents, families, and staff on therapy techniques and home exercise programs.
• Train and support rehabilitation aides as assigned.
• Participate in interdisciplinary meetings and discharge planning.

RESPONSIBILITIES:

• Implement occupational therapy treatment programs under the supervision of an Occupational Therapist
• Assist with resident assessments and contribute to treatment planning within the COTA scope of practice
• Provide therapeutic interventions focused on improving functional abilities, ADLs, mobility, strength, and independence
• Monitor resident progress and communicate findings to the supervising Occupational Therapist
• Complete documentation, care plans, billing, and clinical records accurately and timely
• Educate residents, families, and staff on therapy techniques, adaptive equipment, and home exercise programs
• Train and support rehabilitation aides as assigned
• Participate in interdisciplinary meetings, discharge planning, and quality improvement initiatives
• Ensure residents receive prescribed therapy services and treatment minutes
• Perform additional duties as assigned

REQUIREMENTS:

• Associate of Applied Science Degree from an Occupational Therapy Assistant program approved by the American Occupational Therapy Association
• Valid New York State Certified Occupational Therapy Assistant (COTA) License required
• Newly licensed COTAs welcome to apply
• Skilled nursing facility experience preferred
• Strong communication, documentation, and interpersonal skills
• Ability to work collaboratively with residents, families, therapists, and interdisciplinary team members
• Commitment to providing compassionate, resident-centered care
