Job summary

PACS is seeking a Certified Nursing Assistant to deliver routine daily nursing care and services in accordance with residents' plans and supervision directives. The role involves direct patient care, documentation, and collaboration with nursing staff in a post-acute care setting.

The CNA will identify residents, assist with daily activities, monitor conditions, and maintain a safe, respectful environment. CPR preferred; state licensing required.

Qualifications

• Minimum 10th grade education is required.
• Must be a licensed Certified Nursing Assistant in accordance with state laws. CPR preferred.

Responsibilities

• Identify residents before administering care using wristbands or photo IDs.
• Report changes in resident condition to the Nurse Supervisor/Charge Nurse.
• Record entries on flow sheets, notes, and charts clearly and accurately.
• Maintain resident confidentiality and report unauthorized disclosures.
• Assist residents with daily living activities and personal care.
• Follow facility policies and procedures and cooperate with other staff.
