Job summary

PACS is seeking a Certified Nursing Assistant to provide routine daily nursing care and services to residents as per care plans. The role requires accurate documentation, patient safety, and compassionate support in a post-acute care setting.

Responsibilities include identifying residents, reporting changes in condition, maintaining records, and assisting with daily activities while upholding privacy and safety standards.

Qualifications

• Must possess a CNA license.
• 10th grade education minimum.
• CPR certification preferred.
• Ability to read, analyze, and interpret information.

Responsibilities

• Identify residents before treatments and other care tasks using wristband/photo ID.
• Record entries on flow sheets, notes, charts in a clear, descriptive manner.
• Report changes in residents' condition to Nurse Supervisor/Charge Nurse promptly.
• Maintain confidentiality of residents' protected health information at all times.
• Assist residents with daily functions (dental care, bathing, dressing) and mobility.
• Notify supervisor of absences or lateness and follow facility policies.
