Job summary

Moments Hospice is seeking a dedicated Certified Nursing Assistant (CNA) / Hospice Aide to join our field team. You will provide hands-on personal care, support patients at home or in care facilities, and collaborate with an RN Case Manager to follow care plans, ensuring comfort and dignity in the final chapter of life.

Qualifications

• Active CNA certification required.
• Healthcare caregiving experience in home care or hospice a plus.
• Current CPR certification preferred.
• Experience with electronic medical records.

• Ability to lift 50 lbs and maintain body mechanics.
• Valid driver's license and auto insurance required.

Responsibilities

• Provide ADL care: bathing, dressing, grooming, toileting, skin care.
• Assist with lifting, transfers, positioning, ROM, and mobility.
• Assist with dining and nutrition.
• Review care plans with RN Case Manager.
• Provide comfort and companionship to patients and families.
• Maintain electronic medical records.

Certified Nursing Assistant (CNA) / Hospice Aide

Moments Hospice

Pay: $18 – $23 per hour

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM

Responsibilities

• Provide personal care with activities of daily living, including bathing, dressing, grooming, toileting, and skin care
• Assist with lifting, transferring, positioning, range of motion, exercise, and ambulation
• Assist with dining and nutrition
• Review and follow each patient’s plan of care under the direction of the RN Case Manager
• Provide comfort and companionship to patients and support to their families
• Maintain electronic medical records

Qualifications

• Active Certified Nursing Assistant (CNA) certification (required)
• Healthcare caregiving experience in home care, assisted living/memory care, skilled nursing, hospital, or hospice a plus
• Current CPR certification (preferred)
• Ability to work in an electronic medical record system
• Strong communication skills, including written, verbal, and active listening
• Excellent customer service, problem-solving, and time management skills
• Compassion, patience, and the ability to handle emotional situations with care
• Strong understanding of professional boundaries
• Ability to lift 50 lbs. and good understanding of body mechanics
• Valid driver's license and current auto insurance (required)
