Job summary

Moments Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to provide hands-on personal care to patients in homes, assisted living, or care facilities. You will support daily living activities, monitor comfort, and document care under an RN Case Manager as part of the interdisciplinary team.

This full-time, field-based role offers a pay range of $18–$23 per hour, schedule Monday–Friday, 8:30 AM – 5:00 PM, a fleet car program, company cell phone, benefits, and opportunities to

Qualifications

• Active CNA certification required.
• Experience in home care, assisted living, or hospice a plus.
• Current CPR certification preferred.
• Ability to work in an electronic medical records system.

Responsibilities

• Provide personal care (bathing, dressing, grooming, toileting, skin care).
• Assist with lifting, transfers, positioning, ROM, and ambulation.
• Assist with dining and nutrition.
• Follow patient care plans under RN direction.
• Provide comfort and companionship; support families.
• Maintain electronic medical records.

Pay: $18 – $23 per hour

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM

Responsibilities

• Provide personal care with activities of daily living, including bathing, dressing, grooming, toileting, and skin care
• Assist with lifting, transferring, positioning, range of motion, exercise, and ambulation
• Assist with dining and nutrition
• Review and follow each patient’s plan of care under the direction of the RN Case Manager
• Provide comfort and companionship to patients and support to their families
• Maintain electronic medical records

Qualifications

• Active Certified Nursing Assistant (CNA) certification (required)
• Healthcare caregiving experience in home care, assisted living/memory care, skilled nursing, hospital, or hospice a plus
• Current CPR certification (preferred)
• Ability to work in an electronic medical record system
• Strong communication skills, including written, verbal, and active listening
• Excellent customer service, problem-solving, and time management skills
• Compassion, patience, and the ability to handle emotional situations with care
• Strong understanding of professional boundaries
• Ability to lift 50 lbs. and good understanding of body mechanics
• Valid driver's license and current auto insurance (required)
