Job summary

Moments Hospice is seeking a Certified Nursing Assistant (CNA) / Hospice Aide to provide compassionate, hands-on care to patients in private homes, assisted living, or care facilities. You will work closely with an RN Case Manager in a supportive interdisciplinary team.

The role includes daily care, mobility assistance, nutrition support, and maintaining EMR documentation. Excellent communication, time management, and a caring demeanor are essential; valid driver's license and insurance are

Qualifications

• Active CNA certification required.
• Healthcare caregiving experience in home care, assisted living, or hospice a plus.
• Current CPR certification preferred.
• Ability to work in an electronic medical record system.
• Strong communication and time-management skills.
• Compassion, patience, and ability to handle emotional situations.

Responsibilities

• Provide personal care with activities of daily living, including bathing, dressing, grooming, toileting, and skin care
• Assist with lifting, transferring, positioning, range of motion, exercise, and ambulation
• Assist with dining and nutrition
• Review and follow each patient’s plan of care under the direction of the RN Case Manager
• Provide comfort and companionship to patients and support to their families
• Maintain electronic medical records

Certified Nursing Assistant (CNA) / Hospice Aide

Moments Hospice

Pay: $18 – $23 per hour

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM

• Current CPR certification (preferred)
• Ability to work in an electronic medical record system
• Strong communication skills, including written, verbal, and active listening
• Excellent customer service, problem-solving, and time management skills
• Compassion, patience, and the ability to handle emotional situations with care
• Strong understanding of professional boundaries
• Ability to lift 50 lbs. and good understanding of body mechanics
• Valid driver's license and current auto insurance (required)
