Job summary

Moments Hospice is seeking a dedicated Certified Nursing Assistant (CNA) / Hospice Aide to join our field-based team. You will provide hands-on personal care, help with activities of daily living, and support patients in homes or care facilities under the RN Case Manager's guidance.

Qualifications

• Active CNA certification is required.
• Healthcare caregiving experience is a plus.
• CPR certification is preferred.
• Ability to operate in an electronic medical record system.
• Excellent communication and time-management skills.
• Compassion, patience, and professionalism in handling emotional situations.
• Ability to lift up to 50 lbs.

Responsibilities

• Provide personal care including bathing, dressing, grooming, toileting, and skin care.
• Assist with lifting, transferring, positioning, range of motion, and mobility.
• Assist with dining and nutrition.
• Follow patient care plans under RN supervision.
• Provide comfort and companionship to patients and support to families.
• Maintain electronic medical records.

Certified Nursing Assistant (CNA) / Hospice Aide

Moments Hospice

Pay: $18 – $23 per hour

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM

Qualifications

• Active Certified Nursing Assistant (CNA) certification (required)
• Healthcare caregiving experience in home care, assisted living/memory care, skilled nursing, hospital, or hospice a plus
• Current CPR certification (preferred)
• Ability to work in an electronic medical record system
• Strong communication skills, including written, verbal, and active listening
• Excellent customer service, problem-solving, and time management skills
• Compassion, patience, and the ability to handle emotional situations with care
• Strong understanding of professional boundaries
• Ability to lift 50 lbs. and good understanding of body mechanics
• Valid driver's license and current auto insurance (required)
