Job summary

Moments Hospice is seeking a Certified Nursing Assistant (CNA) to provide hands-on personal care to patients in their homes, ensuring comfort, hygiene, and dignity as part of end-of-life care. You will work closely with an RN Case Manager within an interdisciplinary team, delivering compassionate support in a structured environment with manageable caseloads.

This full-time, field-based role offers a competitive hourly rate and a fleet car program, company cell phone, and comprehensive insurance

Qualifications

• Active Certified Nursing Assistant (CNA) certification is required.
• Healthcare caregiving experience in home care, assisted living/memory care, skilled nursing, hospital, or hospice is a plus.
• Current CPR certification is preferred.
• Ability to work in an electronic medical record system.
• Strong communication, customer service, problem-solving, and time management skills.
• Compassion, patience, and ability to handle emotional situations with care.
• Ability to lift 50 lbs and understand body mechanics.
• Valid driver's license and current auto insurance required.

Responsibilities

• Provide personal care with activities of daily living (bathing, dressing, grooming, toileting, skin care).
• Assist with lifting, transferring, positioning, range of motion, exercise, and ambulation.
• Assist with dining and nutrition.
• Review and follow each patient’s plan of care under RN supervision.
• Provide comfort and companionship to patients and support to families.
• Maintain electronic medical records.

Certified Nursing Assistant (CNA) / Hospice Aide

Pay: $18 – $23 per hour

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
