Job summary

Moments Hospice is seeking a compassionate CNA to provide personal care and support to patients on the road in Wisconsin. You will collaborate with an RN Case Manager, perform daily living activities, and maintain patient records with a focus on comfort and dignity during challenging times.

Qualifications

• CNA license is required to perform duties.

• CPR certification is preferred but not mandatory.

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care).
• Review patient care plans and report changes to the RN Case Manager.
• Assist with lifting and transferring patients safely.
• Assist with range of motion, positioning, exercise and ambulation.
• Assist with dining and nutrition for patients.
• Maintain electronic medical records accurately.
• Work under the direction of a Case Manager RN.

Qualifications:

• Certified Nursing Assistant (CNA) license (required)
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
• Current CPR certification (preferred)
• Excellent customer service skills
• Strong communication (written, verbal, and active listening)
• Problem‑solving skills
• Time management skills
• Ability to work in an electronic medical record system
• Ability to lift 50 lbs.
• Good understanding of body mechanics
• Strong understanding of professional boundaries
• Valid driver’s license (required)
• Valid Auto Insurance (required)
