Job summary

Moments Hospice is seeking a dedicated CNA to provide compassionate in-home care, including activities of daily living, lifting assistance, and mobility support. You will work under an RN Case Manager to ensure patient comfort and dignity during final days.

Qualifications

CNA license required.
12 months caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing, Hospital or Hospice.
Current CPR certification preferred.
Excellent customer service and strong communication skills.
Ability to work with electronic medical records.
Ability to lift up to 50 lbs.

Responsibilities

Perform daily living activities (bathing, dressing, grooming, toileting, skin care).
Review patient care plans.
Assist with lifting/transferring patients.
Assist with ROM, positioning, exercise, and ambulation.
Assist with dining and nutrition.
Maintain electronic medical records.
Work under the direction of an RN Case Manager.

Certified Nursing Assistant (CNA) license (required)
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
