Job summary

Moments Hospice is seeking a compassionate CNA to support patients in their final days. You will perform daily living activities, review care plans, and assist with mobility under the supervision of an RN Case Manager. A strong emphasis on patient comfort and dignity shapes the role.

Qualified candidates hold a CNA license with at least 12 months of caregiving experience, and CPR is preferred. A valid driver’s license and insurance are required to access a company car and electronic medical

Qualifications

• CNA license required.
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required).
• Current CPR certification (preferred).
• Excellent customer service skills.
• Strong communication (written, verbal, and active listening).
• Problem-solving skills.
• Time management skills.
• Ability to work in an electronic medical record system.
• Ability to lift 50 lbs.
• Good understanding of body mechanics.
• Strong understanding of professional boundaries.
• Valid driver’s license (required).
• Valid Auto Insurance (required).

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
