Job summary

Moments Hospice is seeking a compassionate CNA to join our patient-focused team. You will perform personal care tasks, review care plans, assist with transfers, and support mobility and nutrition for patients under an RN Case Manager.

A CNA license and 12 months of caregiving experience are required, with CPR as a plus. We offer a competitive salary, a company car with fuel and insurance coverage, a company cell phone, and comprehensive health benefits.

Qualifications

CNA license is required.
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice is required.
Current CPR certification is preferred.
Excellent customer service skills.
Strong communication (written, verbal, and active listening).
Problem-solving skills.
Time management skills.
Ability to work in an electronic medical record system.
Ability to lift 50 lbs.
Good understanding of body mechanics.
Strong understanding of professional boundaries.
Valid driver's license is required.
Valid Auto Insurance is required.

Responsibilities

Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
Review patient care plans
Assist with lifting and transferring patients
Assist with range of motion, positioning, exercise, and ambulation
Assist with dining and nutrition
Maintain electronic medical records
Work under the direction of a Case Manager RN
