Job summary

Moments Hospice seeks a dedicated Certified Nursing Assistant to provide personal care and comfort on the road. You will work with a Case Manager RN to review care plans, assist with lifting, ROM, nutrition, and daily living tasks, while updating electronic medical records.

Qualifications

CNA license (required).
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required).
Current CPR certification (preferred).

Responsibilities

Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
Review patient care plans.
Assist with lifting and transferring patients.
Assist with range of motion, positioning, exercise, and ambulation.
Assist with dining and nutrition.
Maintain electronic medical records.
Work under the direction of a Case Manager RN.

Qualifications:

Certified Nursing Assistant (CNA) license (required)
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)

Excellent customer service skills
Strong communication (written, verbal, and active listening)
