Job summary

Moments Hospice in Wisconsin is seeking a compassionate Certified Nursing Assistant to provide personal care and companionship to patients in their final days. You will work closely with an RN Case Manager, perform daily living activities, and maintain accurate EMR documentation while delivering dignified care.

Qualifications

• Must hold a valid CNA license and active credentials.
• Minimum 12 months of caregiving experience in relevant settings.
• CPR certification preferred but not required at hire.
• Strong communication and time management skills.
• Ability to use an electronic medical records system.
• Physical ability to lift up to 50 lbs and assist transfers.

Responsibilities

• Perform personal care activities (bathing, dressing, grooming, toileting).
• Review patient care plans and assist with mobility and positioning.
• Assist with meals and nutrition while ensuring comfort.
• Maintain accurate electronic medical records.
• Work under the direction of a Case Manager RN.

Qualifications:

• Certified Nursing Assistant (CNA) license (required)
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
• Current CPR certification (preferred)
• Excellent customer service skills
• Strong communication (written, verbal, and active listening)
• Problem-solving skills
• Time management skills
• Ability to work in an electronic medical record system
• Ability to lift 50 lbs.
• Good understanding of body mechanics
• Strong understanding of professional boundaries
• Valid driver's license (required)
• Valid Auto Insurance (required)

Responsibilities:
