Job summary

Moments Hospice is seeking a dedicated CNA to provide compassionate in-home care, assisting with daily living activities, reviewing care plans, and working under the direction of a Case Manager RN.

This role includes a company car with fuel and insurance, a company cell phone, comprehensive health/dental/vision coverage, generous PTO, and a 401(k) with employer matching. On-call support is available.

Qualifications

CNA license required.
12 months caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing or Hospice.
Current CPR certification preferred.
Excellent customer service skills.
Strong communication, written and verbal.

Responsibilities

Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
Review patient care plans.
Assist with lifting and transferring patients.
Assist with range of motion, positioning, exercise, and ambulation.
Assist with dining and nutrition.
Maintain electronic medical records.
Work under the direction of a Case Manager RN.
