Job summary

Moments Hospice seeks a compassionate CNA to support patient comfort and dignity during their final days. You will perform personal care, review care plans, and assist with mobility under a Case Manager RN, contributing to a meaningful, direct impact on patient well-being.

Qualifications

CNA license required.
12 months caregiving experience in relevant settings.
CPR certification preferred.
Excellent customer service skills.
Strong communication and active listening.

Ability to lift 50 lbs. and use proper body mechanics.
Valid driver's license and auto insurance.
Experience with electronic medical record systems.

Responsibilities

Perform personal care activities (bathing, dressing, grooming, etc.).
Review patient care plans.
Assist with lifting and transferring patients.
Assist with range of motion, positioning, and ambulation.
Assist with dining and nutrition.
Maintain electronic medical records.
Work under the direction of a Case Manager RN.

Qualifications:

Certified Nursing Assistant (CNA) license (required)
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
Current CPR certification (preferred)
Excellent customer service skills
Strong communication (written, verbal, and active listening)

Ability to work in an electronic medical record system
