Moments Hospice in Wisconsin is seeking a compassionate CNA to provide personal care and support under the RN Case Manager, with a strong focus on patient comfort and dignity during final days. This role involves direct patient care, documentation in EMR, and collaboration with a dedicated team.

A competitive wage of $18–$23 per hour plus a company car with fuel and insurance, mobile phone, comprehensive benefits, and immediate PTO accrual.

Qualifications

• CNA license required.
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice.
• Current CPR certification preferred.
• Excellent customer service skills.
• Strong communication (written, verbal, and active listening).
• Problem-solving skills and time management.
• Ability to work in an electronic medical record system.
• Able to lift 50 lbs. and use proper body mechanics.
• Understanding of professional boundaries.
• Valid driver's license and auto insurance required.

Responsabilités

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care).
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
