Job summary

Moments Hospice seeks a compassionate CNA to provide high-quality personal care to patients on the road. You will work closely with a Case Manager RN, support daily living activities, and help maintain patient comfort and dignity in their final days.

Required CNA license with 12 months caregiving experience; CPR preferred. You’ll benefit from a comprehensive package including a company car, health insurance, PTO, and 401(k). Excellent communication and strong time management are essential.

Qualifications

CNA license required and in good standing.
12 months of caregiving experience in home care, assisted living/memory care, skilled nursing, hospital, or hospice.
CPR certification preferred.
Excellent customer service skills.
Strong written and verbal communication, and active listening.
Ability to lift 50 lbs. and use proper body mechanics.
Strong understanding of professional boundaries.
Valid driver's license and auto insurance required.
Experience with electronic medical records.

Responsibilities

Provide personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
Review patient care plans.
Assist with lifting and transferring patients.
Assist with range of motion, positioning, exercise, and ambulation.
Assist with dining and nutrition.
Maintain electronic medical records.
Work under the direction of a Case Manager RN.
