Moments Hospice seeks a compassionate CNA to deliver direct patient care and support in a mobile, on-the-road role. You will work under a Case Manager RN, providing personal care, documenting in EMR, and contributing to patient comfort and dignity.

Qualifikationen

• CNA license required.
• 12 months of caregiving experience in Home Care or related settings.
• CPR certification preferred.
• Excellent customer service skills.
• Strong written and verbal communication and active listening.
• Problem-solving skills.
• Time management skills.
• Ability to work in an electronic medical record system.
• Ability to lift up to 50 lbs.
• Good understanding of body mechanics.
• Strong understanding of professional boundaries.
• Valid driver's license required.
• Valid auto insurance required.

Aufgaben

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
