Job summary

Moments Hospice is seeking a compassionate CNA to provide direct patient care and support under the supervision of an RN Case Manager. You will perform personal care tasks, assist with daily living activities, review care plans, and help with mobility, nutrition, and documentation while traveling on the road.

Candidates should hold a valid CNA license and have at least 12 months caregiving experience in home care, memory care, or hospice.

Qualifications

Certified Nursing Assistant license required.
12 months caregiving experience in relevant settings required.
Current CPR certification preferred.

Responsibilities

Perform daily living activities (bathing, dressing, grooming, toileting, skin care).
Review patient care plans and assist with lifting/transfers.
Assist with range of motion, positioning, exercise, and ambulation.
Assist with dining and nutrition.
Maintain electronic medical records.
Work under RN Case Manager supervision.

Qualifications:

Certified Nursing Assistant (CNA) license (required)
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
Current CPR certification (preferred)
Excellent customer service skills
Strong communication (written, verbal, and active listening)
Problem-solving skills
Time management skills
Ability to work in an electronic medical record system
Ability to lift 50 lbs.
Good understanding of body mechanics
Strong understanding of professional boundaries
Valid driver's license (required)
Valid Auto Insurance (required)
