Moments Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to provide in-home patient care and support. You will work closely with an RN Case Manager to deliver dignified end-of-life care, perform personal care tasks, and maintain accurate electronic medical records.

Formación

• Must have CNA license.
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice.
• CPR certification preferred.
• Excellent customer service and strong communication skills.
• Ability to lift 50 lbs. and understand body mechanics.
• Must have valid driver’s license and auto insurance.

Responsabilidades

• Perform personal care activities (bathing, dressing, grooming, etc.).
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with ROM, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.

Qualifications:

• Certified Nursing Assistant (CNA) license (required)
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
• Current CPR certification (preferred)

• Ability to work in an electronic medical record system
• Ability to lift 50 lbs.
