Zusammenfassung

Moments Hospice is seeking a compassionate CNA to provide personal care and dignity for patients on the road. You’ll support daily living activities, review care plans with a Case Manager RN, and document in the EMR. A company car with fuel and insurance, plus a cell phone, keeps you mobile while you deliver quality hospice care.

Qualifikationen

• CNA license required.
• 12 months caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice.
• CPR certification preferred.
• Excellent customer service and strong communication.
• Problem-solving, time management, and EMR experience.
• Ability to lift 50 lbs and understanding of body mechanics.
• Valid driver's license and auto insurance.

Aufgaben

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care).
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
